  1. Assess whether a hazard will be present, write YES or NO.  Then for each hazard that you have said YES to do the following:
  2. Rate each risk on a Severity scale from Negligable (N), Low risk (L), Moderate risk (M), Severe (S), Very severe (V)

Hazard Number:
3
Description of Hazard:
Audiences
Person(s) exposed (detail if cast/crew (C), outside company (O) or public (P) )
Cast and crew
Action to take:

Action may be required - there will be two of my family members in the house while filming, they will stay out of the way. However in a rare circumstance they could walk in shot and trip over hurting someone or themselves. To avoid this I will put up signs on the door and shut the door when we are filming in particular rooms.

Hazard Number:
25
Description of Hazard:
Stunts, dangerous activities
Person(s) exposed (detail if cast/crew (C), outside company (O) or public (P) )
Cast
Action to take:
Programme for action - for one of my scenes in the house, my character Liam has to drag Luke's dead body. To make sure this is as safe as possible, I will get my actor playing Luke to drag himself along with Liam so that he isn't actually being dragged. It will also be on the grass so it will be soft so neither should injure themselves.

Hazard Number:
32
Description
Slippery floorboards
Person(s) exposed (detail if cast/crew (C), outside company (O) or public (P) )
Cast and crew
Action to take:
Programme for action - the floorboards on location can be quite slippery if you are wearing thick socks. So to make sure no one falls over or trips, I will make sure all my cast and myself wear shoes/trainers inside. This so to avoid that they don't fall over.
